You may use this form to submit anonymous reporting that you feel may not require immediate attention. However, keep in mind that if you have been harassed in any way by another employee or a member or vendor or have concerns about any type of discrimination in the workplace should report the conduct to their direct supervisor or Executive Director immediately.
Confidentiality and Retaliation (104 Employee Handbook): Confidentiality will be protected to the extent reasonably possible. Any employee determined to have violated this policy will be subject to disciplinary action, up to and including immediate termination. Retaliation against an employee who makes a complaint or participating in an investigation is prohibited.
